Social Prescribing Program


We are excited to let you know that Nanaimo has a Social Prescribing Program and all physicians can refer eligible patients!

Social prescribing is an intentional and supportive pathway to connect people to non-medical supports in their community to improve overall health and wellbeing. It complements traditional medical care by addressing the social determinants of health, such as isolation, food insecurity, or housing challenges, with tailored resources like exercise groups, art programs, or peer support networks.

The Nanaimo program is for older adults age 55+ who would benefit from increased social connection in their lives (those with social loneliness, depression, risk of frailty, inactivity, frequent users of primary care).

The below brochure reviews the program and how providers can refer their patients.
